Who is a candidate (Adults vs Teens) to do Invisalign with MSE?

Both adults and teens can benefit from this technique, but we should note that adults will have a four-screw MSE, while teens will have a narrower expander to fit their jaw, which is usually a two-screw MSE. This decision will be made based on the CT scan and the iTero digital scan

Can MARPE / MSE be used with Invisalign?  

Yes, this will require an incredibly careful combination of the MSE / MARPE expander with the progressing Invisalign trays. The MARPE expander will help with widening the maxilla, while the Invisalign trays will help with the teeth movement at the same time. Case #1: MSE with Invisalign in Houston

Here's a case that had braces treatment before, and the crossbite was corrected with a traditional expander. However, the expansion did not hold, and her right side collapsed again after treatment completion. 

Now, she wants to get her bite corrected, but she doesn't want to wear braces again. So she reached out to Dr. Kanaan to see if she could get MSE with Invisalign. Dr. Kanaan created a custom MSE that fit her mouth as well as the Invisalign, and her teeth responded very well according to the plan. 

Her crossbite on the right side was corrected within the planned time, and the patient was extremely happy with no complications. 


Will MSE Create a Gap Between the Front Teeth?

A temporary gap between the upper central incisors may occur during MSE/MARPE expansion. In many cases, this space is a sign that skeletal expansion is occurring. The gap can vary in size depending on the amount of expansion, the patient’s anatomy, and the timing of the aligner sequence.

Case #2
This patient was 22 years old and preferred to avoid braces. He presented with a Class III malocclusion, midfacial deficiency, and a constricted maxillary arch. After a comprehensive examination, Dr. Cannon chose a combined treatment approach using Invisalign with MSE/MARPE.

The MSE/MARPE was used to expand the upper jaw and help mobilize the maxilla, while the patient wore both Invisalign aligners and a facemask at the same time to correct the underbite. The Invisalign trays also served as a bite block, helping to disclude the bite between the upper and lower arches and making the correction more favorable.

As shown in the image, the facemask was successfully incorporated into the treatment plan. This case demonstrates that it is indeed possible to combine facemask therapy, MSE/MARPE, and Invisalign simultaneously in selected adult Class III patients.

Risks, Limits & Alternatives (MARPE, SARPE, Braces)

MSE with Invisalign works well for many patients, but—like any treatment—it has risks and limits. Sometimes the aligner tracking and the jaw expansion don’t match perfectly. This can make the front teeth tilt slightly, or the MSE might over- or under-expand.

To catch these issues early, Dr. Kanaan will ask his patients to send a quick front-teeth photo every 4–6 days so we can make sure the teeth are tracking both to the expansion and to the aligners correctly.

In some cases, the mid-palatal suture may not open as needed; if that happens, a minor surgical aid (piezo) may be recommended—we’ll discuss this during your consultation if needed.

Also, with Invisalign, excellent oral hygiene is essential to prevent cavities or white-spot marks around attachments. You may feel tightness or pressure from the expander and aligners, but over-the-counter pain relief usually helps.
