Yes, we recommend printing and filling out your patient paperwork at home to save time during your visit. If you cannot print them, please arrive 15 minutes early to complete them in our Sugar Land office.

All new patients at iSmile Specialists need to complete the 'Personal, Dental, and Medical Questionnaire' and the 'Consent for Initial Exam and X-Ray' forms.

What should I bring in addition to my completed forms?

Accepted Answer

In addition to your completed patient forms, please bring a valid photo ID and your current dental insurance card so we can verify your benefits during your check-in. *Please note that you can text us your insurance information or a picture of your insurance card. Verfication can take up to 1-2 buisness days to ensure insurance is verified by the time of your appointment please send us that as soon as possible.
