3 Real Class III Cases (Including a “Surgery-Recommended” Case Treated Without Surgery)

I always get a lot of comment regarding my article “underbite correction without surgery”, but recently patients started to ask me: “ Dr. Kanaan, it is great you can fix underbite cases without surgery, but we don’t want braces, can you do it with Invisalign?
Thus, I wrote this article sharing my great experience on how we can correct underbite cases in patients ranging from teenagers to adult patients using invisalign.
While surgery can be appropriate in certain situations, many of the underbites cases can be corrected without surgery using a carefully planned combination of Invisalign® aligners, precise biomechanics, and—in select cases—Temporary Anchorage Devices (TADs).
In this article, I’ll explain how underbite correction with Invisalign clear aligners works, who may be a good candidate for Invisalign, and I’ll share three real cases from our Houston orthodontic practice, including one patient who was considered a surgical candidate but achieved an excellent result without jaw surgery using TADs + Invisalign.

Can Invisalign Fix an Underbite?
Most of the time, Invisalign can correct many underbites when the case is primarily dental (tooth-based), mild to moderate skeletal, or when the goals are compatible with non-surgical correction.
Invisalign underbite correction may involve:
Arch coordination (upper expansion and/or lower constriction within safe limits)
Incisor inclination control (avoiding unstable “flaring” as a shortcut)
Interproximal reduction (IPR) when indicated
Elastics (Class III or cross elastics)
TADs (mini-implants) for anchorage when we need movements that elastics alone cannot deliver predictably
Careful bite opening/settling mechanics

Invisalign’s built-in occlusal coverage can function as an effective “bite-block” (disocclusion) to create clearance for anterior crossbite correction, making underbite mechanics more efficient and predictable.

Underbite primarily dental with mild skeletal influence and cross bite on her right side
Crowding and uneven bite contacts
Treatment goals:
Align teeth, coordinate arches, and correct reverse bite
Maintain or improve gum health and protect enamel from wear
What we did:
We used TADs and Invisalign to correct the underbite by pushing the complete lower arch backward into the wisdom teeth space.
Elastics also were used later to detail the case
Finishing refinement focused on bite settling and midline coordination with asymmetric TADs activation
Outcome:
Improved bite relationship and smile symmetry
Better functional contacts and easier chewing
Why TADs Can Be a Game-Changer for Certain Invisalign Underbite Cases?
TADs / mini Implants are small temporary anchors placed in the gum/bone (typically with local anesthesia). They allow us to:
Apply forces without relying on patient-dependent elastics alone
Improve control of difficult movements (like distalization or intrusion patterns)
Reduce unwanted side effects (like tipping or bite opening in the wrong place)
Create biomechanics that may otherwise require braces or surgery
TADs are not needed for every underbite case, but for the right patient, they can significantly increase predictability.
Case 2:
Male patient from Denver age 19 Y
“Surgery-Recommended” Underbite Corrected Without Surgery (TADs + Invisalign)
Difficulty scale “9/10”
Starting concerns:
Negative overjet (lower front teeth ahead of upper front teeth)
Class III bite skeletal and dental
Patient was advised jaw surgery was the only option elsewhere and wanted a non-surgical solution
What we did:
Invisalign aligners with Dr. Kanaan’s biomechanics plan designed for Class III correction
Strategic use of TAD anchorage to improve control and reduce unwanted tipping
Targeted tooth movements to coordinate arches and correct the underbite relationship
Ongoing monitoring to ensure teeth stayed within healthy bone support
Outcome:
Underbite corrected to a functional bite
Improved incisor relationship and bite stability
Excellent smile improvement without jaw surgery
Total time: 26 months, 77 aligners, 5 refinements as needed
Case 3: 48 Years old Adult Underbite Correction with Invisalign
Starting concerns:
Severe anterior crossbite
Patient wanted an esthetic option vs braces (case selection matters) and does not want to do jaw surgery
Treatment goals:
Correct crossbite/underbite early when stable and indicated
Guide the bite into a healthier relationship and reduce future wear risks
What we did:
Invisalign with a staged approach ( 5 stages ) and 2 TADs with lower placement
Total Treatment time 31 months
Outcome:
Underbite corrected with improved bite guidance
Better alignment and smile display
How We Decide If You’re a Candidate for Invisalign Underbite Correction in Houston

1. Clinical exam (bite function, joints/muscles, gum health) 2. Digital scan for accurate planning 3. X-rays (and CBCT when indicated) 4. A discussion of goals and tradeoffs: non-surgical vs surgical, timeline, retention, and stability
Key candidacy factors include:
Only patient with satisfactory facial appearance will qualify for Dr. Kanaan’s non-surgical technique.
Amount of skeletal discrepancy
Tooth inclinations and available bone support
Presence of crossbites, crowding, missing teeth, or gum recession
How long does Invisalign underbite correction take?
Many cases could be treated in 24 months, but surgical-level discrepancies treated non-surgically may take longer and often require refinements.
Do I need rubber bands with Invisalign for an underbite?
Often, yes. You will need to get 2x TADs as well as Elastics which will be important for bite correction and balance.
Is Invisalign stable for Class III cases?
Dr. Kanaan has a long list of cl III patients treated with Invisalign and TADs who expressed a high stability rate for the skeletal correction. A strong retention plan is essential (often a combination of retainers and, when indicated, fixed retention). In severe cases, a tooth positioner might be needed to keep your teeth and bite perfect.
